Wabash Valley Health Center, Located at 1436 Locust Street, Terre Haute, IN, is a Federally Qualified Health Center (FQHC) providing primary healthcare services.
The clinic sees patients of all ages and accepts most third party payers including Medicaid and Medicare. Wabash Valley Health Center has a sliding fee scale discount available for those that qualify. The discount applied will depend on the number of people who live in the home and the amount of income combined for all living in the home. Household income has to be at or below 200% of the federal poverty level to be considered. The clinic is staffed by a wide array of health care professionals.
Wabash Valley Health Center provides a variety of services, including: Primary Medical Care, Dental Services, Medication Assistance, Behavioral Health, and Outreach & Enrollment.

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

HISTORY
St. Ann Medical Clinic was established on September 9, 1997, providing services one afternoon a week initially. Over time, services were gradually expanded based upon patient needs combined with the availability of fiscal support, equipment, and volunteers. The facility operates in a 110+ year old building that had been used as a Catholic school, a Montessori school, and the St. Ann Parish Hall. The dental clinic opened in 2005.

The facility became incorporated as Wabash Valley Health Center on July 28, 2011. The health center was approved as a 501(c) (3) on June 19, 2012. The clinic continued to operate as a ministry of the Sisters of Providence until early 2015.

The Wabash Valley Health Center received a New Access Point Federal grant on October 30, 2013. This milestone event began the transition from operating for 16+ years as a Free People's Clinic to functioning as a Federally Qualified Health Center. Prior to receipt of the federal grant, the clinic served only those uninsured between the ages of 18 - 64. We now serve all ages and accept most insurance, including Medicaid and Medicare. A sliding fee scale discount is available for those that qualify.
